Parakstot šo medicīnisko sertifikātu, jūrnieks apliecina, ka medicīniskās pārbaudes laikā jūrnieku ārstam ir sniedzis patiesu informāciju par savu veselības stāvokli, ciktāl viņam tā ir zināma, un ir informēts par medicīniskā sertifikāta saturu. (By signing this medical certificate a seafarer is confirming that he (she) has provided true information to the best of his (her) knowledge about the state of his (her) health to the recognized medical practitioner and he (she) is informed of the content of the medical certificate.)
